Primary Care Physician

Physician Responsibilities

• Review patient’s history and perform physical examinations
• Evaluate, diagnose and provide appropriate treatment and patient care
• Collaborate with an interdisciplinary team consisting of specialists, nurse practitioners, RN’s, MA’s, administrative staff, etc.
• Provide value based care services to a panel of members
• Prescribe medications and formulate ongoing treatment and disease management plans
• Oversee clinical staff development and performance
• Help reduce preventable ER visits and hospitalizations

Qualifications

• An active medical license in the state of New York
• Board certified in Internal Medicine, Family Medicine or related specialty
• 1+ year of experience as a Primary Care Physician
• Excellent communication skills
• Interest in Population Health Management
