Primary Care Physician
About the role

The Primary Care Physician (PCP) works as a lead in our team-based care environment, focusing on outpatient medicine, continuity of care, health maintenance, and disease prevention.
Responsibilities
- Works in a team-based care environment, providing proactive, preventive care to seniors.
- Manages a Medicare Advantage panel of patients, understanding Best Practices in coordinated care environments.
- Refers patients to specialists as needed and influences department's strategy.
- Manages medical histories and medical records, making decisions on moderately complex to complex issues regarding technical approaches for project components.
- Communicates effectively with colleagues and patients, demonstrating a high level of interpersonal skills and customer service.
- Adapts to a value-based care setting and learns new practices as needed.
Requirements
- Current and unrestricted medical license or willing to obtain one in state of practice; eligible and willing to obtain licenses in other states in the region of assignment, as required.
- Graduate of accredited MD or DO program from an accredited university.
- Board Certification or Board Eligible in Family Medicine or Internal Medicine (Geriatric Medicine preferred).
- Active and unrestricted DEA license.
- Excellent verbal and written communication skills.
- Highly skilled with interpersonal relationships and communications with colleagues/patients.
- Committed to the Integrated team-based care model.
- Willingness and ability to learn/adapt to practice in a value-based care setting.
- Basic computer skills, including email and EMR.
- Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
- Knowledge of Medicare guidelines and coverage.
- Knowledge of HEDIS quality indicators.
Qualifications
- Minimum of two to five years directly applicable experience preferred.
- Basic computer skills, including email and EMR.
- Experience managing Medicare Advantage panel of patients with understanding of Best Practice in coordinated care environment in a value-based relationship environment.
- Knowledge of Medicare guidelines and coverage.
- Knowledge of HEDIS quality indicators.

- Total compensation package (base pay + bonus) could exceed $300K, depending on experience and location.

Pay
The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $255,000 - $298,500 per year.
