Primary Care Physician
Conviva Primary Care, a subsidiary of Humana Inc., is seeking a primary care physician for our clinic in Plantation, Florida. You will take a hands-on approach, addressing patient needs inside the exam room while also understanding how social determinants of health affect health outcomes. About the role
As a Primary Care Physician, you will report to our Associate Medical Director and focus on outpatient medicine, continuity of care, health maintenance, and disease prevention. Responsibilities include maintaining medical histories and records, referring patients to specialists, and influencing clinic department strategy. You will make decisions on moderately complex to complex issues with considerable latitude in determining the best approaches to assignments.
Schedule
- Monday–Friday, 8:00 AM–5:00 PM
- 40 scheduled weekly hours
- Join an experienced team with an established patient panel
- See 12–15 patients per day with longer authorized appointments (45 minutes)
- Spend 100% of your time clinically focused on direct patient care, including patient-facing and related administrative time
Responsibilities
- Provide outpatient primary care with a focus on continuity of care, health maintenance, and disease prevention
- Maintain medical histories and records
- Refer patients to specialists as needed
- Influence clinic department strategy
- Make decisions on moderately complex to complex issues
Requirements
- Doctor of Medicine or Osteopathic Medicine
- Board certified or board eligible in Geriatric Medicine, Family Medicine, or Internal Medicine
- Valid, unrestricted medical license in the state of desired employment (or willingness to obtain)
- Current, unrestricted DEA license
Preferred Qualifications
- Minimum of two or more years of experience
- 1+ years of experience managing a Medicare Advantage panel of patients
- Knowledge of Medicare guidelines and coverage
- Knowledge of HEDIS quality indicators
- Bilingual (Spanish and English) a plus
This role is patient-facing and part of Humana/Senior Bridge's Tuberculosis (TB) screening program. If selected, you will be required to undergo TB screening.
Pay
The pay range reflects a good faith estimate of starting base pay for full-time (40 hours per week) employment at the time of posting. Geographic location and individual pay will vary based on demonstrated job-related skills, knowledge, experience, education, and certifications.
- Base pay: $255,000 – $298,500 per year
- Eligible for a bonus incentive plan based on company and/or individual performance
