About the role
Conviva Primary Care, a subsidiary of Humana Inc., is seeking a primary care physician for their clinic in Deland, FL. Reporting to the Associate Medical Director, this position involves being onsite at the clinic from Monday to Friday, 8:00 AM to 5:00 PM. A typical day includes seeing 12-15 patients daily, with longer authorized appointments of up to 45 minutes.
Responsibilities
- Provide proactive, preventive care to seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment.
- Manage a Medicare Advantage panel of patients.
- Address the social, emotional, behavioral, and financial needs that can impact patients' well-being.
Requirements
- Doctor of Medicine or Osteopathic Medicine degree.
- Board certified or board eligible in Geriatric Medicine, Family Medicine, or Internal Medicine.
- Valid, unrestricted license in the state of desired employment (or willingness to obtain).
- Current, unrestricted DEA license.
- Screened for TB.
- Minimum of two years of experience in value-based care.
- One to two years of experience managing Medicare Advantage panel of patients.
Qualifications
- Minimum of two years of experience in value-based care.
- One to two years of experience managing Medicare Advantage panel of patients.
- Associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency.

- Strong interpersonal and communication skills.
