Responsibilities
- Demonstrates accountability for outcomes, strong clinical care, and cost-effectiveness for each patient in their panel of up to 450 patients.
- Acts as a health coach, helping patients set short and long-term health goals, and working with them to achieve those goals.
- Simplifies and prioritizes appropriately to make behavior change more actionable for both the patient and the care team.
- Works closely with hospitalists and other health care professionals to ensure compliance with guidelines and participate in risk and quality management programs.
- Engages with the hospitalist when patients are in the hospital, regardless of whether the hospitalist works for ChenMed or not.
- Assesses, diagnoses, treats, manages, educates, promotes health and coordinates care for patients with acute and complex chronic health needs.
- Leads a care team consisting of a medical assistant, care facilitator, and care coordinator for patients who can come to the office. Engages with the transitional care team and other resources for patients who cannot come to the office.
- Takes an active role in recruiting patients for the center and additional providers for the company.
- Supervises, collaborates with, participates with, or functions within a practice or collaborative agreement with an Advanced Practice Practitioner (APP).
- Adheres to departmental goals, objectives, standards of performance, regulatory compliance, quality patient care compliance, and policies and procedures.
Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board Eligibility is required once Board certified.
- Maintains board certification in their specialty by doing necessary MOC, CME and/or retaking board exams as required.
- Current State Medical License for the state(s) in which he/she will be working.
- Current DEA number for schedule II-V controlled substances.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment.
