Primary Care Physician
About the role
The Primary Care Physician (MD/DO) is a key member of our interdisciplinary care team. In the role, you will evaluate our patients’ acute and chronic conditions, manage their multiple medications, mental health, socio-emotional needs, and provide empathetic patient education. The PCP will work closely with fellow clinical teammates to provide comprehensive primary care. While direct patient care is the primary focus of the position, our PCPs also assist with development, implementation and achievement of key organizational objectives and metrics. Functioning as a value-based care model, our team of clinicians proactively and strategically manage our population’s health in accordance with IHI’s Quintuple Aim, adopt best practices in coding compliance and achieve our HEDIS quality metrics.
Responsibilities
- Evaluate patients’ acute and chronic conditions
- Manage patients’ multiple medications, mental health, socio-emotional needs
- Provide empathetic patient education
- Work closely with fellow clinical teammates to provide comprehensive primary care
- Avoid direct patient care responsibilities
- Avoid assisting with development, implementation and achievement of key organizational objectives and metrics
- Avoid functioning as a value-based care model
- Avoid proactively and strategically managing our population’s health in accordance with IHI’s Quintuple Aim
- Avoid adopting best practices in coding compliance
- Avoid achieving HEDIS quality metrics
Requirements
- Licensed MD/DO with board certification with MBPS recognition (IE ABFM, ABIM, etc)
- Minimum 3-5 years of experience managing complex disease longitudinally over time including diabetes, cardiovascular disease, menopause, thyroid disease, osteoporosis, weight management, etc.
- Understanding or willingness to learn how to practice in a VBC model-including managing the cost and quality of care
- Experience managing overall patient care and devising individualized care plans
- Experience educating and counseling patients on health maintenance and disease prevention
- Experience with geriatric medicine, particularly women
- Experience with poly-pharmacy management and appropriate medication reduction
- Experience with physical and gynecological/well-woman examinations in the elderly population
- Experience working in clinical situations that required adaptability
- Strong problem-solving and critical thinking skills
- Up-to-date knowledge of medical advancements and technologies
