Primary Care Physician
About the role
The Primary Care Physician (MD/DO) is a key member of our interdisciplinary care team. In the role, you will evaluate our patients' acute and chronic conditions, manage their multiple medications, mental health, socio-emotional needs, and provide empathetic patient education. The PCP will work closely with fellow clinical teammates to provide comprehensive primary care. While direct patient care is the primary focus of the position, our PCPs also assist with development, implementation, and achievement of key organizational objectives and metrics.
Responsibilities
- Evaluate patients' acute and chronic conditions, manage their multiple medications, mental health, socio-emotional needs, and provide empathetic patient education.
- Work closely with fellow clinical teammates to provide comprehensive primary care.
- Assist with development, implementation, and achievement of key organizational objectives and metrics.
- Proactively and strategically manage patients' health in accordance with IHI's Quintuple Aim, adopt best practices in coding compliance, and achieve HEDIS quality metrics.
- Manage the cost and quality of care in a value-based care (VBC) model.
- Provide clinical procedures within scope of practice (skin biopsies, joint injection, endometrial biopsies).
- Manage overall patient care and devise individualized care plans.
- Educate and counsel patients on health maintenance and disease prevention.
- Manage the care of patients with complex diseases such as diabetes, cardiovascular disease, menopause, thyroid disease, osteoporosis, weight management, etc.
- Work in clinical situations that require adaptability and strong problem-solving and critical thinking skills.
- Have up-to-date knowledge of medical advancements and technologies.

Requirements
- Licensed MD/DO with board certification in Family Medicine (ABFM) or Internal Medicine (ABIM).
- Experience managing complex diseased longitudinally over time including diabetes, cardiovascular disease, menopause, thyroid disease, osteoporosis, weight management, etc.
- Understanding or willingness to learn how to practice in a value-based care (VBC) model, including managing the cost and quality of care.
- Experience managing overall patient care and devising individualized care plans.
- Experience educating and counseling patients on health maintenance and disease prevention.
- Experience with geriatric medicine, particularly women.
- Experience with poly-pharmacy management and appropriate medication reduction.
- Experience with physical and gynecological/well-woman examinations in the elderly population.
- Experience working in clinical situations that required adaptability.
- Strong problem-solving and critical thinking skills.
- Up-to-date knowledge of medical advancements and technologies.

Qualifications
- Eligibility to obtain or holder of a DEA License and an NPI number.
- Regular maintenance of board certification and CME requirements.
- Passage of the pre-employment background check.
- Completion of registration/credentialing with Medicare and private payers.
- Valid state-issued Driver’s License.
- Proof of US Citizenship or Eligibility to Work in the US with a permanent resident card.
Skills
- Technical skills with EMR (Athena preferred) and Microsoft Suite.
- Experience in mentoring, precepting, and/or supervision of other provider staff.

The compensation for this position is commensurate with experience.
Schedule
The employment type is Full Time, Exempt (30-40 hours per week).
