Responsibilities
- Deliver comprehensive primary care at home/senior communities visits to patients
- Represent Bloom Healthcare in the St Louis market as a Bloom ambassador supporting our new market growth including market analysis to understand the local healthcare landscape and competitors.
- Strategic planning to grow your patient panel and deliver comprehensive primary care at home visits to 10 patients per day.
- Build trusted relationships by engaging with key members of our patients’ care teams, including assisted living facilities, hospitals, clinics, and community organizations, to facilitate coordination of care.
- Collaborate with interdisciplinary teams, including specialists, pharmacists, and social workers, to coordinate care, address complex patient needs, and achieve optimal health outcomes.
- Provide data-driven, condition-specific specialty visits to patients as needed
- Manage the health outcomes of an empaneled patient population
- Engage with and appropriately utilize population health data to drive superior patient outcomes
- Attend ongoing educational opportunities
- Occasional travel to Denver market for initial and ongoing training
- Scheduled Days: Monday - Friday (3, 4, or 5 days)
- Relocation assistance packages available
- Flexible schedules with rotating call (2-3x yr)

- Competitive salary range of $225,000 - $260,000 annually in the state of Missouri

Pay
The salary for this position is competitive and commensurate with experience. The pay range for this role in the state of Missouri typically falls between $225,000 - $260,000 annually. Actual compensation may vary based on factors such as qualifications, experience, and location within the state.
