About the role
Deliver comprehensive, outcome-focused care to a panel of up to 450 patients, primarily older adults.
Responsibilities
- Provide diagnosis, treatment, management, and care coordination for acute and chronic conditions.
- Lead care teams, collaborate with specialists, and engage in patient health coaching to promote behavior change.
- Monitor key metrics such as patient admissions, experience, and clinical gaps, working towards organizational goals.
- Ensure compliance with standards, participate in clinical meetings, and foster a positive, accountable culture within the center.
Qualifications & Skills
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar;
- active State Medical License;
- Board eligibility or certification preferred;
- DEA license;
- BLS certification;
- proficiency in Microsoft Office;
- excellent communication skills;
- ability to work independently and collaboratively;
- willingness to travel locally and nationally as needed.
