Role Overview

The Primary Care Physician (PCP) provides comprehensive, outcome-driven care to a panel of up to 450 patients, focusing on acute and chronic illnesses, especially in older adults. They act as health coaches, promoting behavior change, and work collaboratively with care teams to improve patient health outcomes.

Key Responsibilities

• Assess, diagnose, treat, and manage patient health needs, ensuring quality and compliance.
• Lead and coordinate care teams, including care promoters and facilitators, especially for patients unable to visit the office.
• Engage with hospital and transitional care teams for hospitalized or homebound patients.
• Maintain accountability for patient outcomes, meet clinical and operational metrics, and foster a positive, accountable center culture.
• Support patient engagement, recruitment, and provider coverage as needed.

Qualifications & Skills

• MD or DO with certification or eligibility in Internal Medicine, Family Medicine, Geriatrics, or related fields.
• Active state medical license, DEA registration, and BLS certification.
• Proficiency in evidence-based medicine, leadership, and communication skills.
• Comfortable with independent judgment, technology, and local travel.

Additional Requirements

• Commitment to continuous professional development and maintaining board certification.
• Fluent in English; additional languages a plus.

Pay

This role offers a salary range of $214,700 to $306,714, with comprehensive benefits, career growth opportunities, and an emphasis on quality patient care and team leadership.
