Primary Care Physician

About the role

The Primary Care Physician (PCP) is responsible for delivering comprehensive, outcome-focused care to a panel of up to 450 patients, primarily older adults. The PCP acts as a health coach, guiding patients in setting and achieving health goals, while simplifying care plans for better adherence. They lead care teams, coordinate with specialists, and manage acute and chronic illnesses, ensuring high-quality, cost-effective care. Accountable for patient outcomes, clinical care, and adherence to regulatory and quality standards.

Responsibilities

• Collaborates with care teams, hospitalists, and specialists, especially for hospitalized or high-need patients.
• Engages in clinical assessment, diagnosis, treatment, education, and care coordination.
• Supervises and partners with Advanced Practice Practitioners, promoting a positive practice culture.

Requirements

• Possesses MD or DO in relevant fields, active medical license, board eligibility, BLS certification, and proficiency in Microsoft Office.
• Requires strong communication skills, independence, and willingness to travel locally as needed.
