Responsibilities
- Provide comprehensive primary care to a diverse patient population
- Collaborate with interdisciplinary care teams
- Perform in-office procedures (optional)
- Participate in quality and population health initiatives
- Educate and engage patients in their care
Requirements
- Unrestricted medical licensure in the state of Indiana or able to obtain prior to start
- Board certified or board eligible in Family Medicine, Internal Medicine or Med-Peds
- Active and unrestricted DEA license or ability to obtain prior to start
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree
- Completion of an accredited ACGME/ACGME-approved residency program in Family Medicine, Internal Medicine, or Medical-Pediatrics
- Passing scores on the USMLE or COMLEX examinations
