Responsibilities
- Provide comprehensive primary care to a diverse patient population
- Collaborate with interdisciplinary care teams
- Perform in‑office procedures (optional)

Required Qualifications
- Unrestricted medical licensure in Indiana or ability to obtain prior to start
- Board certified or board eligible in Family Medicine, Internal Medicine, or Med‑Peds
- Active and unrestricted DEA license or ability to obtain prior to start
Pay
The salary range for this role is $226,000 to $366,000 per year. Salary range is defined as total cash compensation at target. The actual range and pay mix of base and bonus is variable based upon experience and metric achievement. Pay is based on several factors including, but not limited to, local labor markets, education, work experience, and certifications.
