About the Role

Work in a smaller, collegial office caring for adult patients. This is a full-time APP position where you will have the opportunity to build your own panel of patients. 40-hour work week, Tuesday through Friday. Experienced or new graduates welcome to apply.

Hospital site: Bridgeport. Location: Trumbull, CT. EMR system: Epic.

Responsibilities

Clinical Care

• Provide assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team
• Work in collaboration with the attending supervising/collaborating physician(s) in accordance with state law and hospital regulations, hospital policy, protocols, and clinical pathways; may provide autonomous team-based care
• Obtain medical histories, perform physical examinations, review/order diagnostic/therapeutic tests and radiologic results, and formulate/manage patient treatment plans
• Perform procedures, prescribe treatment and medications in accordance with institutional and state guidelines, as well as specialty or professional competency standards
• Function as first or second assistant on operative cases (within scope and job educational prerequisites), using fine motor skills and manual dexterity
• Respond to emergency situations within YNHHS parameters

Patient Education & Psychosocial Support

• Educate patients/families on care and recommended or available resources, including admitting instructions, treatment plans, side effects of treatments/medications, and discharge plans
• Provide psychosocial support to patients, family members, and significant others as part of a multidisciplinary team

Information Management

• Ensure appropriate documentation in accordance with hospital and regulatory requirements
• Protect information in all forms (computer-based, paper, film, voice, or other media) from unauthorized access, modification, destruction, or disclosure

Quality Management

• Demonstrate high standards of moral and ethical behavior, professionalism, compassion, and commitment to patient care
• Ensure compliance with patient-related healthcare regulatory requirements, standards, and recommendations

Professional Development

• Engage in projects that maintain and advance professional competency
• Serve as a clinical resource for new house staff and provide service-specific orientation
• Demonstrate high-reliability organizational behaviors through reporting safety events and near misses
• Maintain all employee requirements determined by hospital policy
• Serve as a clinical preceptor for NP/PA students and other healthcare professionals, and mentor pre-PA/NP candidates
• Collaborate on research, quality assurance, and performance improvement projects
• Assist with the formation of protocols applied to clinical practice or further educational and clinical expertise

Requirements

• Graduate from an accredited Nurse Practitioner or Physician Assistant Program
• APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process
• Evidence of current competence to practice via appropriate reference letters from physicians and other practitioners must be obtained during the Medical Staff appointment process

Qualifications

Licensure

For Physician Assistants:

• Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA)
• Connecticut State Physician Assistant License
• ACLS/BLS Certified (when indicated for position profile)
• DEA License (when indicated for pediatric position profile)
• PALS (when indicated for neonatal care)
• NRP Certification
• Additional licensure, certifications, and/or continuing education credits may be required during employment based on position and area of specialty

For Nurse Practitioners:

• Connecticut State Registered Nurse License
• Connecticut State Nurse Practitioner License
• Board specialty certification (as appropriate for position profile)
• ACLS/BLS Certified (when indicated for position profile)
• DEA License (when indicated for pediatric position profile)
• PALS (when indicated for neonatal care)
• NRP Certification
