About the Role

Work in a smaller, collegial office caring for adult patients. This is a full-time APP position where you will have the opportunity to build your own panel of patients. 40-hour work week, Tuesday through Friday. Experienced or new graduates welcome to apply.

Responsibilities

Clinical Care

• Provide assessment, triage, and therapeutic management of patient/family needs throughout the disease process as part of an interdisciplinary team
• Work in collaboration with the attending supervising/collaborating physician(s) in accordance with state law and hospital regulations, protocols, and clinical pathways
• Obtain medical histories, perform physical examinations, review/order diagnostic/therapeutic tests and radiologic results, and formulate/manage patient treatment plans
• Perform procedures, prescribe treatments and medications in accordance with institutional and state guidelines
• Function as first or second assistant on operative cases (within scope and job educational prerequisites)
• Respond to emergency situations within YNHHS parameters

Patient Education & Psychosocial Support

• Educate patients/families on care, treatment plans, side effects, and discharge plans in collaboration with the supervising/collaborating MD
• Provide psychosocial support to patients, family members, and significant others as part of a multidisciplinary team

Information Management

• Ensure appropriate documentation in accordance with hospital and regulatory requirements
• Protect patient information from unauthorized access, modification, destruction, or disclosure

Quality Management

• Demonstrate high standards of moral and ethical behavior, professionalism, compassion, and commitment to patient care
• Ensure compliance with patient-related healthcare regulatory requirements, standards, and recommendations
• Report safety events and near misses consistently

Professional Development

• Engage in projects to maintain and advance professional competency
• Serve as a clinical resource for new house staff and provide service-specific orientation
• Maintain all employee requirements determined by hospital policy
• Serve as a clinical preceptor for NP/PA students and other healthcare professionals
• Mentor pre-PA/NP candidates
• Collaborate on research, quality assurance, and performance improvement projects
• Assist with the formation of protocols to enhance clinical practice

Requirements

Education

• Graduate from an accredited Nurse Practitioner or Physician Assistant Program

Experience

• Clinical working experience as an NP/PA within the given specialty/service line recommended but not required

Licensure & Certification

For Physician Assistants:

• Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA)
• Connecticut State Physician Assistant License
• ACLS/BLS Certified (when indicated for position profile)
• DEA License (when indicated for pediatric position profile)
• PALS (when indicated for neonatal care)
• NRP Certification

For Nurse Practitioners:

• Connecticut State Registered Nurse License
• Connecticut State Nurse Practitioner License
• Board specialty certification (as appropriate for position profile)
• ACLS/BLS Certified (when indicated for position profile)
• DEA License (when indicated for pediatric position profile)
• PALS (when indicated for neonatal care)
• NRP Certification

Additional licensure, certifications, and/or continuing education credits may be required during employment based on position and area of specialty.

APRNs, PAs, CRNAs, and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process. Evidence of current competence to practice must be obtained during the Medical Staff appointment process.
