Primary Care Nurse RN/LPN (Hybrid) - Mercy Clinics Fort Smith

About the role

Under the direction of the primary care operations team, the Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards.

Qualifications

• Education: Graduate of an accredited registered professional nursing program.
• Licensure: RN license within the hiring state and/or compact licensure in which nursing duties are performed; must meet all state board of nursing requirements.
• Physical requirements/demands: Ability to push, pull, and/or lift 50 pounds on a regular basis; prolonged standing and walking each shift; ability to grip, reach, bend, kneel, twist, and squat to perform duties.
• Certifications: Basic Life Support (BLS) certification through the American Heart Association or successful completion within 30 days of hire; one or more of the following may be required based on position/unit or acquired within department required timeframe: ACLS, NRP, ENPC, TNCC, CPHON, OCN, BLS, PALS, PEARS, S.T.A.B.L.E. Six Assessment & Care Modules, C-EFM, and other unit‑specific certifications as required.
