About the role
The Primary Care Nurse works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Nurse will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for an attributed panel. Duties and responsibilities will be performed in a manner consistent with Mercy's mission, values, and Mercy Service Standards.
Responsibilities
- Collaborate with primary care providers to deliver patient-centered care.
- Partner with patients, providers, and the clinical care team to drive quality outcomes.
- Perform duties in alignment with Mercy’s mission, values, and service standards.
Requirements
- Graduate of an accredited registered professional nursing program.
- RN license within the hiring state and/or compact licensure in which nursing duties are performed; must meet all state board of nursing requirements.
- Basic Life Support (BLS) certification through the American Heart Association or completion within 30 days of hire.
- One or more of the following certifications may be required based on the position/unit or within a department-specified timeframe:
- ACLS (Advanced Cardiac Life Support)
- NRP (Neonatal Resuscitation Program)
- ENPC (Emergency Nursing Pediatric Course)
- TNCC (Trauma Nursing Core Course)
- CPHON (Certified Pediatric/Hematology/Oncology Nurses)
- OCN (Oncology Certified Nurse)
- PALS (Pediatric Advanced Life Support)
- PEARS (Pediatric Assessment Recognition and Stabilization)
- S.T.A.B.L.E. (Sugar, Temperature, Airway, Blood Pressure, Lab Work, And Emotional Support) Six Assessment & Care Modules
- C-EFM (Fetal Monitor Certification)
- Other unit-specific certifications as required.
Physical Requirements
- Regularly push, pull, and/or lift 50 pounds.
- Prolonged standing and walking each shift.
- Ability to grip, reach, bend, kneel, twist, and squat to perform duties.
