The Primary Care Nurse collaborates with providers and the care team to deliver patient-centered, safe, and efficient healthcare within a primary care setting. They partner with patients and team members to drive quality outcomes for an attributed panel, ensuring alignment with mission, values, and standards.

Responsibilities

• Provide direct nursing care in a primary care environment.
• Support clinical workflows and ensure efficient operations.
• Participate in onsite training during an initial period.

Qualifications

• Active RN licensure.
• Diploma in an Associate of Applied Science in Nursing (required).
• Bachelor of Science in Nursing (preferred).
