About the role

The Primary Care Nurse collaborates with providers and the clinical team to deliver safe, effective, patient-centered care in a systematic manner, aiming to improve quality outcomes for an assigned patient panel.

Responsibilities

• Partner with patients, providers, and team members to ensure equitable and efficient care
• Work in alignment with organizational standards and values
• Provide direct patient care and support clinical workflows
• Contribute to quality improvement efforts
• Perform responsibilities in accordance with mission and service standards

Requirements

• Active RN licensure
• Diploma in Associate of Applied Science in Nursing

Qualifications

A Bachelor of Science in Nursing is preferred.

Schedule

Hybrid/remote setting with initial onsite training and onsite work two days per week.
