Position Summary

The Nurse Practitioner (NP) at Upstate Family Health Care Inc. provides health services on an outpatient basis to patients of Upstate Family Health Center. The NP works collaboratively with physicians to manage health care needs, educates and guides patients, and coordinates services.

Essential Roles and Responsibilities

  • Assess the health status of patients through health and medical history.
  • Provide continuity of healthcare to patients, ensuring understanding and compliance with therapeutic regimens.
  • Work with patients to understand and adhere to therapeutic regimens within established protocols and refer clients to supervising physicians or other healthcare providers as necessary.
  • Provide instruction and counseling to individuals, families, and groups in areas of health promotion, health education, and maintenance.
  • Collaborate with other health care providers and agencies to provide and coordinate services to patients and families.
  • Maintain clinical records according to established protocols.
  • Maintain clinical competence through continuing education, certification, and licensure.
  • Participate in planning programs and developing health policies and procedures.
  • Practice safety, environmental, and infection control methods.
  • Attend department meetings and mandated continuing education/training programs.
  • Comply with all pertinent regulatory and agency regulations and operating standards.
  • Create a positive work environment that fosters communication between supervisors and employees and integrates new employees into the Agency's culture, values, and mission.
  • Participate in team activities such as meetings, trainings, and committees.

Education And Experience Requirements

  • Family Nurse Practitioner License (FNP) in the State of New York
  • Experience preferred, training will be provided to chosen candidate
  • 3+ years of experience
  • Evidence Of Essential Leadership, Communication, Education, And Counseling Skills
  • Proficiency in communication technologies (email, cell phone, etc.)
  • Highly organized with ability to keep accurate notes and records
  • Local knowledge about the connections to community health care and social welfare resources is desirable
  • Working knowledge of PCMH and the current requirements
  • Knowledge Skills And Abilities (KSAs) Requirements
  • Ability to work collaboratively with the healthcare team
  • Ability to work effectively with individuals or groups in a teaching situation
  • Ability to design and conduct research
  • Knowledge of health education, practice, and theory
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community
  • Ability to maintain quality, safety, and infection control standards
  • Knowledge of clinical operations and procedures
  • Knowledge of current principles, methods, and procedures for the delivery of medical evaluation, diagnosis, and treatment in area of expertise
  • Knowledge of related accreditation and certification requirements
  • Intermediate Computer Skills
  • Must possess the ability to make independent decisions

  • Acknowledges patient’s rights on confidentiality issues, maintains patient confidentiality at all times, and follows HIPAA guidelines and regulations

Physical Demands

  • Certain deadlines and unanticipated developments may require work during evenings, weekends.
  • Ability to quickly address any emergent issues without losing focus on task at hand.
  • The employee must have full sight and hearing with fluency in the English language.
  • While performing the duties of this job, the employee is regularly required to talk or hear.
  • The employee frequently is required to stand, walk, and sit.
  • Must be able to sit for long periods of time.
  • Constantly operates a computer and other office productivity machinery, such as a copy machine, scanner, computer printer, etc.
