Position Highlights & Primary Responsibilities
- Serve as the first clinical point of contact for patients entering the Adult and Family Medicine practice
- Focus on new patient visits, problem-focused evaluations, and care navigation, with appropriate transition to long-term primary care providers
- Assist with acute and subacute concerns, chronic disease assessment and stabilization, and initiation of preventive care
- No procedural responsibilities beyond standard outpatient IM/FM scope
- Anticipated patient volume of 14-18 patients per day, based on visit type and complexity
- 20-30-minute visit lengths, with flexibility for more complex visits
- Work in a highly collaborative, team-based environment working closely with IM physicians, APCs, nursing staff, and medical assistants
- Key access role supporting timely patient evaluation and panel growth
- Monday-Friday, daytime schedule
- No call, weekend, or holiday responsibilities

Required Qualifications
- Board Certified as a Nurse Practitioner (NP) or Physician Assistant (PA)
- Unlimited Licensure in the state of Massachusetts
- Active DEA License
- Minimum of 3 years experience as an APC
Preferred Qualifications
- Experience working within a Team Care model
- Fluency in EMR systems & technology
- EPIC experience a plus
Salary Range
The salary range for this role is $109,500 to $164,000 annually based on full-time employment. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.
