Role Overview
We're seeking experienced Primary Care Nurse Practitioners to provide on-site care to residents in Assisted Living and RCFE communities across the Inland Empire. If you're passionate about geriatric care and want to make a meaningful difference in the lives of seniors — without the walls of a clinic — this role is for you.
What You'll Do
- C Conduct routine and urgent assessments; manage chronic disease across a dedicated patient panel
- B Perform post-discharge follow-ups, medication reconciliation, and coordinate labs and diagnostics
- C Collaborate with facility staff, specialists, families, and POAs to ensure seamless continuity of care
- D Complete same-day documentation in compliance with CMS and regulatory standards
- E Serve as the primary on-site clinical provider, building lasting relationships with residents and care teams
Schedule & Setting
Part-time position with a flexible weekday schedule. Providers deliver on-site care across the Inland Empire with strategically clustered routes — so you spend more time with patients and less time on the road.
Requirements
- A Active California NP and RN licenses; active furnishing license
- B Valid NPI, DEA, and BLS certification
- C Bilingual fluency in Spanish and English required
- D Experience in geriatrics, assisted living, RCFE, or facility-based primary care preferred (strongly preferred)
- E EMR proficiency and commitment to same-day documentation
- F Comfortable working independently in a mobile, field-based environment
Benefits
- A Annual compensation: $85,000 - $100,000 (based on experience, schedule, and patient volume)
