Primary Care Nurse or LPN - Full-Time

We are seeking a Primary Care Nurse to collaborate with providers and the clinical team in delivering patient-centered, safe, and effective outpatient care. Responsibilities include managing triage calls/messages, supporting quality outcomes, and working in a systematic manner aligned with organizational standards. Qualifications Vary By Role LPN: Diploma in Practical Nursing, active license, 2-3 years of preferred experience.RN: Associate degree or higher in Nursing, active RN license, 2-3 years of preferred experience. Skills preferred include proficiency with Epic EMR, excellent triage and computer skills, time management, and adaptability. Position offers a hybrid work model post-training, with on-site responsibilities and optional remote work, requiring attendance at monthly meetings.
