Seeking a Primary Care Nurse to collaborate with providers and the clinical team, ensuring safe, effective, patient-centered care in a systematic approach. Responsibilities include managing triage calls/messages, driving quality outcomes, and partnering with patients to meet their healthcare needs. Qualifications Vary By Role For LPNs, a Diploma in Practical Nursing, active LPN license, and 2-3 years of experience are preferred. For RNs, an Associate's degree, BSN, or equivalent experience, along with an active RN license and 2-3 years of experience, are required. Key skills include proficiency with Epic EMR, triage, computer competency, time management, and adaptability. Work schedule is Monday to Friday, 8:00 AM to 4:30 PM, with a hybrid model after initial training, including on-site collaboration.
