Collaborate with providers and the clinical team to deliver patient-centered, efficient, and equitable care within an outpatient clinic setting. Partner with patients, manage timely triage, and drive quality outcomes for an assigned panel, all aligned with the organization’s mission and standards.

Requirements
- Licensed Practical Nurse (LPN): Diploma and active license; 2-3 years of experience preferred
- Registered Nurse (RN): Associate or Bachelor’s degree and active license; 2-3 years of experience preferred, or equivalent experience
Skills
- Proficiency with Epic EMR
- Strong triage and computer skills
- Excellent time management and adaptability
Schedule
Full-time hours with a hybrid work model after initial training. No nights, weekends, or holidays.
