Primary Care Assistant - MA - Sullivan Clinic

About the role

Under the direction of the primary care operations team, the Primary Care Patient Care Advanced MA works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Patient Care Advanced MA will partner with patients, providers, and other members of the clinical care team to drive quality outcomes for all patients. Duties and responsibilities will be performed in a manner consistent with our mission, values, and Mercy Service Standards.

Responsibilities

• Complete Advanced Standard Rooming with overall achievement ≥ 90% for the last 6 months, including AWV questionnaire and documentation, thorough medication reconciliation, closing all open care gaps as indicated, encounter guide (Fall Risk, Depression, Breast Cancer Screening, Colorectal Cancer Screening, Diabetes Eye Exam), blood pressure recheck for patients with elevated initial blood pressure reading, and completion of patient experience conversation.
• Provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way.
• Partner with patients, providers, and other members of the clinical care team to drive quality outcomes for all patients.

Requirements

• High school diploma or equivalent
• Preferred: graduation from an accredited Medical Assistant program
• Preferred: one year of applicable patient care experience or applicable medical office experience as defined by the clinic
• Must attend MA Clinic Academy training (new hire)
• Must have completed all MA competencies
