Role: Licensed Practical Nurse (LPN) in a primary care setting, working under the supervision of the primary care operations team. Responsibilities: Collaborate with providers and the care team to deliver safe, effective, patient-centered care; support quality outcomes; perform duties aligned with organization standards and values. Qualifications: Graduate of an accredited practical nursing program; valid LPN licensure in the practice state; preferred 6 months clinical experience, ideally in primary care.
