About the role

CHASI’s Prevention Department focuses on outreach, education, and referrals for priority populations to improve overall health and wellbeing and decrease acquisition of HIV and STIs through comprehensive strategies. Navigators play a critical role by addressing barriers clients face in accessing services, providing case management and referrals to ensure comprehensive HIV prevention services.

Responsibilities

  • Conduct intakes, mental health, and SUD screenings and other assessments to identify client needs
  • Educate and counsel clients on prevention strategies and materials based on their presentation and needs
  • Develop, coordinate, and integrate service plans in collaboration with clients
  • Assist with appointment scheduling, provider follow-ups, logistics, reminders, transportation, and accompany clients to appointments as needed
  • Perform case management for assigned caseloads from intake to discharge, tracking progress and maintaining detailed notes
  • Provide appropriate referrals to internal and external services; build relationships with partner providers
  • Ensure successful linkage, coordination, and follow-up with providers, including PrEP, PEP, HIV care, and STI treatment
  • Support clients in connecting with appointments and action plans, addressing barriers
  • Support outreach events and community engagement activities, including weekends as needed
  • Conduct home visits and escort clients to appointments as needed
  • Collect and submit accurate data on persons contacted and engaged in services
  • Maintain updated and accurate records of services and activities provided
  • Participate in documentation quality assurance and reconciliation tasks
  • Keep weekly updates on program client lists and deliverable trackers
  • Participate in training, professional development, and assigned meetings
  • Perform all functions aligned with CHASI’s Mission, Vision, and Core Values

Requirements

  • At least 2 years of experience providing case management or health education with individuals at high risk of contracting HIV/AIDS, or with histories of mental illness, substance use, immigrant or LGBTQ+ populations
  • High School diploma or equivalent required; some college preferred
  • Bilingual (English/Spanish) preferred
  • Basic knowledge of HIV, HCV, STI, PEP/PrEP, and Harm Reduction methods
  • Preferred experience with PrEP AP, Gilead, and applying for copay cards
  • Excellent MS Office skills (Outlook, Word, Excel, PowerPoint)
  • Ability to work independently and collaboratively in diverse teams with shared responsibility and accountability
  • Professionalism and tact in interacting with internal and external partners
  • Strong verbal and written communication skills, including listening, following instructions, giving direction, and following up on tasks
  • High attention to detail and accuracy; highly organized and able to multitask

$41,600–$46,600 annually

Schedule

Full-time; on-site; may include weekends and occasional nights
