About the Role

The Pre-Visit Coordinator plays a critical role in supporting healthcare delivery by coordinating pre-visit assessments prior to home health visits. This position ensures accurate and current member information is documented through telephonic communication with providers, members, and families. The role directly contributes to the quality and efficiency of care coordination in a high-volume, remote call center environment.

Responsibilities

  • Conduct inbound and outbound telephonic outreach to members using auto-dialer and manual dialing
  • Complete Pre-Visit Assessments by gathering demographic and medical history information
  • Communicate effectively with members and providers to ensure access to primary care providers
  • Identify and document gaps in care, preventive screenings, allergies, and surgeries
  • Maintain high-quality customer service and meet established metrics for quality, productivity, and outcomes
  • Adhere to schedule and attendance requirements
  • Protect confidential health information in compliance with HIPAA and other regulations

Requirements

  • High School Diploma, GED, or equivalent experience
  • Minimum of 18 years of age
  • At least 2 years of call center and/or telephonic customer service experience, preferably in healthcare
  • At least 2 years of healthcare, social work, community outreach, or advocacy experience
  • Minimum of 1 year documenting medical history using medical terminology
  • Proficiency with Microsoft Word, Excel, and Outlook
  • Availability to work full-time Monday through Friday, 9:45 am to 6:15 pm CST, with occasional overtime or weekends

Preferred Qualifications

  • Experience as Licensed Practical Nurse, Certified Medical Assistant, Community Health Worker, or Certified Nursing Assistant
  • Experience working with Medicare or Medicaid members
  • Prior telecommuting experience
  • Ability to work onsite if needed due to technical issues
  • Bilingual in English and Spanish

Telecommuting Requirements

  • Residence within Eastern, Central, or Mountain Time Zones
  • Dedicated, private workspace with secure handling of sensitive documents
  • Reliable wired high-speed internet connection meeting specified standards

Skills

  • Comfortable in fast-paced, high-volume call center settings
  • Ability to meet or exceed departmental metrics and goals
  • Proficient multitasking between phone and computer systems
  • Strong organizational skills and telephone etiquette
