Practitioner (bachelor's level)

About the role

You will complete the Wheels of Change and field training to become a certified FCT and FCT-R Practitioner. The FCT-R Practitioner will provide in-home mental health and/or substance use treatment with the intent to keep families together, based on the Family Centered Treatment Model. You will support clients and families on their mental health and/or recovery journey by collaborating with a team of practitioners and Peer Recovery Specialists to promote recovery, stability, and the ability to maintain the family within the home. This is a hybrid role with 20% remote work.

Responsibilities

• Work with clients and their family members and caregivers to identify opportunities and goals, assess functioning, develop a treatment plan, and assist in achieving desired outcomes.
• Build and maintain positive relationships with clients, family members and caregivers, and identify referring or collateral professionals and agencies.
• Collaborate with collateral contacts as needed.
• Provide two 2-hour treatment sessions per week with each client/family assigned, with flexibility to meet client needs.
• Maintain accurate client records and complete documentation requirements in a timely manner.

Requirements

• Bachelor’s degree in social work or related field required; Master of Social Work degree preferred.
• Minimum 1 year experience in a similar role required; experience with trauma-informed and substance use treatment a plus.
• On-call availability as needed.
• Statewide travel is required.
• Must authorize Child & Family to complete a motor vehicle records check.
• Must have a valid driver's license, auto insurance, and car registration.
• Must meet all background checks and medical clearances required for the position.

Skills

• Strong organizational, critical thinking, and time-management skills.
• Basic computer skills; knowledge of Microsoft Office 365.
• Ability to communicate and effectively interact with a diverse population.
• Bilingual in Spanish preferred.
• Collaboration and teamwork.

Physical Requirements

• This position requires time sitting, standing, walking, and driving.
