About the role

The Adoption Program at Lund seeks a Post Adoption Case Manager, passionate about supporting families formed through adoption. In alignment with Lund’s mission and values, the role promotes family stability and resiliency through individualized parent coaching and support that honors each family’s unique story. The position educates parents on early trauma, attachment, and the lifelong needs associated with adoption, while ensuring access to adoption-competent providers and practitioners. Advocating for families in school and team meetings, organizing and facilitating monthly support groups, conducting home visits and connecting caregivers with training and community resources are key components of this work.

Responsibilities

  • Educate parents on early trauma, attachment, and the lifelong needs associated with adoption
  • Ensure access to adoption-competent providers and practitioners
  • Advocate for families in school and team meetings
  • Organize and facilitate monthly support groups
  • Conduct home visits
  • Connect caregivers with training and community resources

Requirements

  • Bachelor’s Degree in Human Services field
  • Preferred 2 years of professional experience in trauma and adopt-related work or an equivalent of transferrable professional and lived experience
  • Strong writing skills, organizational, interpersonal and team skills
  • Ability to focus on multiple tasks
  • Ability to work independently and be self-motivated
  • Ability to accept challenge and adapt to change
  • Ability to maintain a professional relationship with a variety of people
  • Ability to maintain a valid VT Driver’s License and access to reliable transportation is required; occasional statewide travel may be required
