Job Summary
MedOne is seeking a Wound Certified Advanced Practice Practitioner for its Post-Acute team in Columbus. This position is part-time, working 20 hours per week.
Responsibilities
- Evaluates new and existing patients by performing a history and physical and interpreting laboratory and diagnostic studies for complex wound patients on the MedOne Service.
- Works in collaboration with a physician in developing treatment plans for wounds and follow-up.
- Orders and interprets diagnostic and laboratory tests related to the patient’s needs.
- Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
- Maintains patient and family confidentiality, privacy, and patient rights.
- Documents in the patient record as appropriate according to MedOne and institution guidelines.
- Maintains clinical privileges through adherence to credentialing and privileging processes.
- Bills or logs encounters as directed.
- Communicates treatment plan between patient, family, and the medical team.
- Facilitates the patient’s transition within and between health care settings such as admitting, transferring, and discharging patients.
- Completes wound notes within 24 hours.
- Acts as a role model and resource to other advanced practice clinicians and nursing staff.
- Be willing to see home health wound patients.
Qualifications
- Wound Certified Advanced Practice Practitioner certification.
- Experience in post-acute wound care.
- Knowledge of wound care treatments and modalities.
- Strong communication and collaboration skills.
- Ability to work independently and as part of a multidisciplinary team.
