  Responsibilities

  • Serves as primary liaison, teacher, coach and facilitator for practice transformation for practices participating in the network.
  • Provides onsite and telephonic practice visit coaching using a standardized approach and demonstrates the ability to effectively lead and facilitate improvement teams towards measurable outcomes.
  • Maintains a supportive and collegial role with the primary care team and collaborates to meet provider and practice needs.
  • Conduct practice needs assessment to learn about practice demographics, daily operations and current workflows.
  • Supports providers and practice teams on office based care delivery interventions resulting in cost of care savings and improved health outcomes for patients.
  • Shadow, educate and/or train physicians and medical staff as necessary in clinical workflows and coordination of care to improve clinical quality and achieve VBC goals.
  • Obtains and analyzes quality metrics and reports for care opportunities, supports practice implementation of care coordination and care management, identifies action plans for providers to improve cost, quality and patient experience and participates in design, development and implementation of community learning forums.
  • Provides education for practices to develop expertise with metrics and data review for quality improvement.
  • Works with CCP leadership on communication to physicians regarding available patient care programs through VBC programs—both written/email communication and visits to provider meetings as necessary.
  • Reviews reports from payers and assists management in interpretation of reports and actions needed.
  • Tests and pilots new VBC initiatives and works with CCP leadership on refining processes, helping achieve contractual metrics.
  • Assists in the development, implementation and monitoring of corporate and/or HealthONE policies and procedures as they relate to VBC programs.
  • Performs other duties as assigned by supervisor.

  Requirements

  • B.A./B.S. in a health care related field coupled with 5 years of experience in primary care, ambulatory care or other related field.
  • Commensurate experience in healthcare, specifically VBC programs, may take the place of the B.A./B.S. requirement.
  • Preference given for a Master’s degree, and/or a strong background in one or more of the following: VBC programs, quality or process improvement, data analysis, healthcare recognition/accreditation, change management and personnel management.
  • Valid U.S. driver’s license, personal vehicle and insurance required.
  • Ability to analyze payer quality data and navigate reports from various Electronic Health Records.
  • Excellent computer skills required, particularly related to Microsoft applications Word, Access, Visio, Excel, PowerPoint and Outlook.
  • Comfort with exploring and learning to use new or unfamiliar applications or databases.
  • Working knowledge of health information technology and ability to connect practices to necessary resources to produce needed data/reporting.
  • Managed Care Understanding specifically ACO/Value Based Contracts.
  • Public speaking centered around medical practice education.
  • Commitment to collaboration, professionalism, and effective communication in all interactions with physicians, HealthONE Physician Services employees, patients, caregivers and payers.
