Nurse Practitioner (Full Time) Regional Traveler - In-Home Health Assessments

About the Role

Join a comprehensive in-home health assessment team as a Nurse Practitioner (NP) or Advanced Practice Provider.

Responsibilities

• Conduct up to one-hour assessments of adult and geriatric members in their homes or facilities.
• Review health history, medications, social factors, and risks to improve care quality and identify undiagnosed conditions.
• Administer tests and collaborate with physicians, case managers, social workers, and caregivers.
• Provide follow-up and referrals.

Requirements

• Master’s Degree or equivalent with NP licensure and prescriptive authority.
• Board certification in relevant specialties (e.g., Adult Gerontology, Family, Emergency).
• At least 1 year of NP experience.
• Current BLS/ACLS/CPR certifications.
• Ability to travel extensively.
• Valid driver’s license and automobile insurance.
• Strong communication skills.
