Population Health and Wellness Care Management Coordinator

Role Overview: The Population Health/Wellness Case Management Coordinator (CMC) supports healthcare outcomes through advocacy, collaboration, education, and care management tools. This telework position involves regional travel up to 50%, requiring reliable transportation, a valid KY driver’s license, and occasional office visits. Responsibilities: Conduct member assessments, review data, and recommend care approaches to maintain health and well-being. Identify high-risk factors and implement proactive interventions. Collaborate with multidisciplinary teams to enhance care quality, resolve barriers, and escalate issues as needed. Utilize negotiation and motivational interviewing skills to engage members, promote lifestyle changes, and empower decision-making. Monitor, evaluate, and document care in compliance with regulations and policies. Qualifications: Minimum of a Bachelor's or master's degree in behavioral health or a relevant human services field. At least 2 years’ experience in behavioral health or social services, with knowledge of foster care, juvenile justice, trauma-informed care, and related programs preferred. Proficiency in computer systems, MS Office, and remote work setup is required. Flexibility to work beyond standard hours is necessary.
