Pod Provider (MD/DO)

Cityblock Health, a premier value-based care organization, is seeking a Physician for Ohio, based in either Columbus or Cincinnati. This role reports to the Vice President of Market Medical Practice and focuses on serving our community’s most complex members with comprehensive, whole-person care that integrates physical health, behavioral health, and social support.

About the role

The Cityblock Pod Provider serves as a practice-based provider, supporting members with scheduled appointments, walk-ins, and urgent or emergent visits across multiple modalities (in-person or virtually; in a hub, community, or home setting). You will deliver comprehensive chronic disease management (e.g., hypertension, diabetes, chronic kidney disease), evaluate acute concerns, manage behavioral health conditions (e.g., depression, anxiety, alcohol and substance use disorder), and provide preventative care and wellness support, including sexual and reproductive health.

The role works collaboratively with an interdisciplinary team of clinical and non-clinical members to develop member care plans and support members throughout their care journey. You will also assist in developing and enhancing clinical programs, initiatives, processes, and policies while managing the most complex patients in your market.

Responsibilities

• Clinical Visits
  • Conduct comprehensive clinical intake visits for selected members based on segmentation, program empanelment, and care team recommendations.
  • Provide timely follow-up for members post-transition of care, acute needs, or condition-specific requirements.
  • Proactively outreach to members who need follow-up care.
  • Deliver clinical care in line with Cityblock Health protocols.
  • Complete comprehensive and compliant diagnosis capture related to HCC (Hierarchical Condition Categories) and ICD-10 coding.
  • Address contractual and company-prioritized quality gaps, ensuring proper chart documentation and codes (ICD or CPT) are included as evidence of gap closure.
  • Respond to member needs and delegate tasks in a timely fashion.
  • Finish documentation within 48 hours of clinical encounters, per company policy.
• Clinical Scope
  • Prescribe 1st and 2nd line medications for depression, anxiety, alcohol use disorders, and opioid use disorders.
  • Continue psychiatric medications for members with stable serious mental illness (SMI).
  • Remain up-to-date on evidence-based care for complex patients in primary care (adult only, pediatric in certain markets).
  • Provide direct primary care or wrap-around care with a focus on chronic condition management and preventative care.
  • Refer members to specialist care appropriately, following internal Cityblock guidance.
  • Support members in achieving the goals outlined in their care plans (physical, behavioral, and/or social health).
• Case Review and Care Planning
  • Participate in case conferences with the care team, providing clinical expertise on care plan development.
  • Offer guidance on the highest-impact clinical interventions and tasks to achieve a member’s goals.
• Panel Management
  • Collaborate on a panel of members to provide clinical input, including health maintenance, chronic disease management, and co-occurring psychiatric disorder support.
  • Work with the care team to determine the need for member placement in different programs (e.g., lower or higher intensity).
• Operations
  • Utilize care facilitation, electronic health record, and scheduling platforms to document member interactions, organize information, track tasks, and communicate with your team, members, and community resources.
• Leadership
  • Educate, support, and elevate team members’ skills through informal coaching.
  • Act as the collaborating physician for oversight of Advanced Practice Clinicians.
  • Complete chart reviews in compliance with Cityblock policy.
  • Guide the clinical team on complex cases and be available for escalation questions at the point of care.
  • Provide clinical consultation for care team members, including support with care plans and review of clinically concerning member conditions.
• Supervision
  • Manage Advanced Practice Clinicians (APCs) supervision assignments and maintain Collaborative Practice Agreements (CPA), ensuring high-quality oversight and collaborative care.

Requirements

• 3+ years of direct clinical care experience.
• Bachelor’s degree.
• Graduate of an accredited medical college (within the United States/Canada) or an unaccredited medical college (outside the United States/Canada).
• Current valid license to practice medicine in Ohio.
• Board certification in a major primary care specialty field of practice.
• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a similar specialty.
• Certified in Basic Life Support for Healthcare Providers.
• Must complete Cityblock’s medical clearance requirements, including evidence of immunity to MMR, Hepatitis B, Varicella, and a TB screen, or have an approved medical or religious accommodation.

Pay

The expected salary range for this position is $206,250.00 - $275,000.00 annually. Final compensation will be determined based on qualifications, skillset, experience, and geographic location.
