Role Overview

Part-Time Psychiatric Mental Health Nurse Practitioner, Fredericksburg, VA. We are seeking an experienced and collaborative Psychiatric Mental Health Nurse Practitioner (PMHNP) to support our Fredericksburg team on a part-time basis. This role offers the opportunity to provide high-quality, person-centered psychiatric and mental health care within post-acute, long-term care and assisted living settings.

Key Responsibilities

  • Provide comprehensive psychiatric mental health assessment, diagnosis and treatment to patients in post-acute, long-term care and assisted living settings
  • Conduct initial, routine and follow-up psychiatric evaluations, including review of mental health history, symptoms, medications, diagnoses, psychosocial needs and current concerns
  • Assess, diagnose and treat a range of psychiatric and behavioral health conditions within the scope of practice
  • Evaluate mood, cognition, behavior, thought processes, risk factors, functional status and changes in mental health presentation
  • Develop, implement and review individualized psychiatric treatment plans in collaboration with patients, families, facility staff and interdisciplinary care teams
  • Prescribe, review and manage psychotropic medications in accordance with applicable regulations, clinical guidelines and patient needs
  • Monitor treatment response, therapeutic outcomes, adverse effects, drug interactions, polypharmacy and clinical risk
  • Support appropriate use, monitoring and reduction of psychotropic medications when clinically indicated
  • Manage changes in psychiatric status, behavioral symptoms, acute mental health needs and urgent patient concerns, escalating care appropriately when required
  • Order, review and follow up on relevant laboratory tests, diagnostic investigations and other clinical information
  • Collaborate with physicians, nurse practitioners, nurses, psychologists, therapists, social workers, specialists, hospitals and community mental health resources
  • Participate in care planning meetings, interdisciplinary reviews, family consultations and facility clinical meetings as required
  • Support appropriate transitions of care, hospital follow-up and coordination of psychiatric and behavioral health services across the healthcare continuum
  • Provide education and support to patients, families and facility staff regarding diagnoses, medications, treatment plans, symptom management, behavioral health and crisis prevention
  • Promote evidence-informed psychiatric practice, patient safety, trauma-informed care, suicide prevention, infection prevention, risk management and quality improvement
  • Maintain accurate, timely and clinically appropriate documentation in the Electronic Medical Record (EMR)
  • Communicate effectively with the supervising or collaborating physician and other members of the care team, as applicable

Qualifications And Required Skills

  • Master's degree or Doctor of Nursing Practice (DNP) from an accredited psychiatric mental health nurse practitioner program
  • Current, unrestricted Virginia Psychiatric Mental Health Nurse Practitioner license and eligibility to practice in Virginia
  • Current national board certification as a Psychiatric Mental Health Nurse Practitioner
  • Current prescriptive authority and Drug Enforcement Administration (DEA) registration, as required
  • Experience in post-acute, long-term care, assisted living, geriatric psychiatry, outpatient psychiatry, primary care, hospital or related clinical settings preferred
  • Experience providing psychiatric and behavioral health care to older adults and patients with complex medical needs preferred
  • Strong psychiatric assessment, diagnostic reasoning, risk assessment and treatment planning skills
  • Knowledge of psychopharmacology, geriatric psychiatry, dementia-related behaviors, chronic mental health conditions and care coordination
  • Knowledge of evidence-informed approaches to anxiety, depression, bipolar disorder, psychosis, substance use, trauma-related conditions and behavioral symptoms
  • Proficiency with Electronic Medical Record systems and timely, accurate documentation practices
  • Excellent communication, collaboration, organization and problem-solving abilities
  • Compassionate, culturally sensitive, trauma-informed and person-centered approach to patient care
  • Ability to work independently while collaborating effectively with physicians and multidisciplinary care teams
  • Availability to provide reliable part-time PMHNP coverage based on patient and facility needs
