Role Overview: Part-Time Psychiatric Mental Health Opportunity, Charlottesville, VA We are seeking an experienced and collaborative Psychiatric Mental Health Nurse Practitioner (PMHNP) to support our Charlottesville team on a part-time basis. This role provides high-quality, person-centered psychiatric assessment, diagnosis and treatment within post-acute, long-term care and assisted living settings. The successful candidate will evaluate mental health and behavioral concerns, develop and manage individualized treatment plans, monitor treatment effectiveness, support medication management, coordinate care with interdisciplinary teams and promote safe, effective continuity of care. discounted professional liability insurance where applicable Key Responsibilities Provide comprehensive psychiatric mental health assessment, diagnosis and treatment to patients in post-acute, long-term care and assisted living settingsConduct initial, routine and follow-up psychiatric evaluations, including review of mental health history, symptoms, medications, diagnoses and current concernsAssess and manage a range of psychiatric, psychological and behavioral health conditions within the scope of practiceEvaluate mood, affect, thought processes, cognition, behavior, functional status, safety risks and changes in clinical presentationDevelop, implement and review individualized psychiatric treatment plans in collaboration with patients, families, facility staff and interdisciplinary care teamsPrescribe, review and manage psychiatric medications in accordance with applicable regulations, clinical guidelines and patient needsMonitor treatment response, therapeutic outcomes, adverse effects, drug interactions, polypharmacy and clinical riskAssess and manage changes in mental health status, acute psychiatric needs, behavioral concerns and urgent patient issues, escalating care appropriately when requiredEvaluate patients for suicidal ideation, self-harm, aggression, psychosis, delirium and other safety concerns, initiating appropriate interventions and referralsOrder, review and follow up on relevant laboratory tests, diagnostic investigations and other clinical information related to psychiatric treatmentCollaborate with physicians, nurse practitioners, nurses, psychologists, therapists, specialists, hospitals and community mental health resourcesParticipate in care planning meetings, interdisciplinary reviews, family consultations and facility clinical meetings as requiredSupport appropriate transitions of care, hospital follow-up and coordination of psychiatric and behavioral health services across the continuumProvide education and support to patients, families and facility staff regarding diagnoses, medications, treatment plans, coping strategies, symptom management and prevention of clinical deteriorationPromote evidence-informed psychiatric practice, patient safety, trauma-informed care, infection prevention, risk management and quality improvementMaintain accurate, timely and clinically appropriate documentation in the Electronic Medical Record (EMR)Communicate effectively with the supervising or collaborating physician and other members of the care team, as applicable Qualifications And Required Skills Master’s degree or Doctor of Nursing Practice (DNP) from an accredited Psychiatric Mental Health Nurse Practitioner programmedCurrent, unrestricted Virginia Psychiatric Mental Health Nurse Practitioner license and eligibility to practice in VirginiaCurrent national board certification as a Psychiatric-Mental Health Nurse Practitioner through the appropriate certifying organizationCurrent prescriptive authority and Drug Enforcement Administration (DEA) registration, as requiredExperience in psychiatric mental health, post-acute, long-term care, assisted living, geriatric, outpatient, primary care, hospital or related clinical settings preferredExperience caring for older adults and patients with complex psychiatric, medical and behavioral health needs preferredStrong psychiatric assessment, diagnostic reasoning, risk assessment and treatment planning skillsKnowledge of psychopharmacology, chronic mental health conditions, dementia-related behavioral symptoms, substance use concerns and care coordinationProficiency with Electronic Medical Record systems and timely, accurate documentation practicesExcellent communication, collaboration,
