About the role

The CHI Case Manager classification is utilized in the Public Health Department. Incumbents are responsible for assisting families enrolled in the Child Health Initiative services for children age 0 to 18. Incumbents report to the program’s coordinator.

Responsibilities

  • Conducts telephone contacts with families to assess enrollment status in the Child Health Initiative program and helps resolve any problems between the family, the health coverage program administrator and their health plan;
  • Informs and encourages individuals and/or families on the significance and appropriate use of preventive health care;
  • Provides assistance to individuals and/or families in overcoming any barriers to utilization of services, such as setting appointments, changing health plans/providers, or retaining health coverage;
  • Gathers and maintains documentation of individual and/or family use and access to health care services;
  • Works to ensure program enrollment and re-enrollment;
  • Provides education and conducts presentations to individuals and families about program services and enrollment;
  • Advocates on behalf of the individual and/or family to ensure re-determination packets are received and services are utilized;
  • Assists individuals and/or families by providing referral and enrollment services to other available health coverage programs;
  • Aids in the completion of appropriate health coverage enrollment and re-determination forms;
  • Utilizes automated eligibility verification system or Medi-Cal database system;
  • Maintains regular contact with enrollees and their families, either in person or by telephone;
  • Prepares and generates correspondence;
  • Gathers and maintains program statistics and prepares reports;
  • Attends meetings and other trainings as authorized;
  • Travels frequently within the County to contact enrollees and families.

Requirements

  • Education: Completion of 30 (semester) college units with course work in liberal arts, business, human services or a related field.
  • Experience: One year of experience providing outreach, education, case management, or health care services, preferably involving providing enrollment and/or eligibility assistance to individuals or families.
  • Licenses: Possession of a valid California driver’s license issued by the Department of Motor Vehicles.

  • Develop and maintain positive relationships with community groups, agencies, health service providers and individuals;
  • Safely operate a motor vehicle.
