Responsibilities

• Deliver outpatient primary care to senior members
• Focus on building relationships with patients through small panel sizes
• Lead care teams with a patient-centric orientation
• Address both medical and social needs of patients
• Provide care for those with comorbid conditions, the elderly, and/or medically underserved populations

Requirements

• M.D. or D.O. graduate
• Board certified/eligible in Internal Medicine or Family Medicine (required)
• Fellowship training in Geriatrics welcome, but not required
• Active, non-probationary state medical license
• Highly collaborative with a passion for patient-centered care
• Confidence in delivering high-quality primary care
• Driven to care for patients with complex medical needs
• Energized by working in a fast-growing organization
