Responsibilities
- Deliver high-quality primary care to a primarily Medicare-aged, medically underserved patient population with comorbid conditions
- Manage a caseload of patients, ensuring continuity of care and addressing both medical and social needs
- Conduct thorough examinations and diagnostics as needed
- Collaborate with a multidisciplinary team to provide comprehensive care
- Be available to patients and address their medical and social needs
- Participate in ongoing education and training to maintain and enhance clinical skills
Requirements
- Currently licensed to practice professional nursing or Physician Assistant in the state of employment, active and non-probationary
- Experienced in primary care: internal medicine, geriatrics, or family medicine
- Current BLS/ACLS certification
- Strong understanding of examination methodologies and diagnostics
- Able to demonstrate interpersonal skills to be approachable and understandable to patients, families, and center team members
- Confident in their ability to deliver high quality primary care
- Comfortable traveling within the market to multiple centers within short distance of each other
Qualifications
- Master's degree in Nursing or Master's or Doctoral degree in Physician Assistant Studies
- Minimum of 5 years of relevant experience in primary care
- Experience working with Medicare-aged patients and those with comorbid conditions
- Experience collaborating with a multidisciplinary team
