Physician - Primary Care

About the role

The mission of VA Northern Indiana Health Care System (VANIHCS), is to provide coordinated, compassionate and comprehensive quality primary care, mental health, surgical, intensive and extended care to Veterans in a timely, accessible and cost effective manner. It is a fully-integrated medical care facility with a dual campus and combined VA mission.

Duties

• Provides quality patient-centered care to adults age 18-65 and older adults age 65+, as assigned by ACOS of Primary Care.
• Affords assessments by utilizing advanced clinical judgment and physical assessment skills. Identifies acute/non-acute/chronic health problems.
• Ensures safe, efficient, and high quality healthcare delivery within the department.
• Ensures appropriate initial evaluation, treatment, and consultation, as well as documentation, consistent with current VHA Directive and VHNIHCS policies on Emergency Medicine.
• For admissions to the inpatient units, ensures a physician-to-physician conversation regarding the need for admission, including diagnoses, assessment and recommendations.
• Demonstrates sound decision-making skills, both clinical and administrative, which bring credit to the profession and serve to enhance the distinctive and contributory role of physicians.
• Daily care of patients also entails regular interaction with nursing, coordination of consultations and care and facilitation of diagnostic studies and therapeutic procedures.
• Other vital roles include: communication with patients and their families, patient education, discharge planning and communication with outpatient providers, arranging follow up, preparing discharge instructions, medication reconciliation, and ensuring communication during hand-offs of care.
• Ensures a sign-out to the oncoming physician to ensure an informed transition of care; coordinate with the admitting physician and consultants to organize diagnostic and treatment plans for all patients pending admission.
• Maintains appropriate clinical privileges in order to fulfill the duties of assignment within Primary Care Service.

Qualifications

• U.S. Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
• Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
• Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
• Proficiency in spoken and written English.
• Primary Care experience preferred.
