About the Role
Provides direct patient care in accordance with evidence-based treatment guidelines at Deer Valley Medical Center’s Orthopedics department. Delivers care in a team-centered, Patient-Centered Medical Home model, focusing on both well care and chronic disease management to improve patient health. Engages patients and families in healthcare decisions, provides wellness and prevention education, and makes appropriate referrals for specialty care and training.
Responsibilities
- Provide direct patient care following evidence-based guidelines in a Patient-Centered Medical Home model.
- Manage chronic disease states and deliver well care using a team-centered approach.
- Engage patients and families in healthcare decisions and direction.
- Offer counseling, education, and resources on wellness, prevention, and health maintenance.
- Refer patients to specialty care services and educational training as needed.
- Use disease registries to identify care gaps and improve healthcare metrics through support services and care managers.
- Improve patient access to care by participating in process improvement for same-day appointments and extended hours.
- Enhance care delivery efficiencies in the office setting to utilize “top of license” care by the team.
- Maintain accurate and timely documentation of patient encounters to meet coding guidelines, CMS, HIPAA, and other regulations.
- Work with the Practice Development and Improvement Manager to achieve Patient-Centered Medical Home status and NCQA certification.
- Participate in clinical integration, including process improvement, care coordination, and transition of care to specialists and inpatient services.
- Ensure timely follow-up after inpatient discharge for smooth transition to outpatient care.
- Act in a fiscally responsible manner, delivering value (quality/cost) to patients by providing the right care at the right time.
- Regularly attend meetings and events.
Requirements
- M.D. or D.O. degree (Required).
- Member of HonorHealth medical staff (Required).
- Bilingual (Spanish) preferred.
- Basic Life Support (BLS) certification (Required).
- Board Certified or eligible (Required).
- Current State of Arizona or other compact state medical license (Required).
- Fingerprint Clearance Card (Required for Pediatric Behavioral Health Sites only).
