Responsibilities

  • Provides direct patient care in accordance with evidence-based treatment guidelines.
  • Delivers well care and manages chronic disease states in a team-centered approach.
  • Interacts with patients and families to keep them informed and engaged in their health care decisions.
  • Provides counseling, education, and resource information on wellness, prevention, and health maintenance.
  • Refers patients as appropriate for specialty care services and educational training.
  • Uses disease registries to identify gaps and out-of-range values in care delivery.
  • Participates in improving access to care through process improvement initiatives.
  • Works with the process team to enhance care delivery efficiency in the office setting.
  • Maintains, monitors, and ensures accurate and timely documentation of patient encounter records.
  • Collaborates with Practice Development and Improvement Manager on achieving Patient-Centered Medical Home status and NCQA certification.
  • Engages in clinical integration, including process improvement, coordination of care transitions, and communication with specialists and inpatient services.
  • Follows up with patients post-discharge to ensure a smooth transition from inpatient to outpatient care.
  • Strives to deliver value (quality/cost) to patients by providing timely and appropriate care.

Requirements

  • M.D. or D.O. - Required
  • Member of HonorHealth medical staff - Required
  • Bilingual-Spanish - Preferred
  • Basic Life Support (BLS) - Required
  • Board Certified or eligible - Required
  • Physician\MD - Physician - State Licensure - Required
  • Fingerprint Clearance Card - State - Required
