Responsibilities
- Provides direct patient care in accordance with evidence-based treatment guidelines.
- Delivers well care and manages chronic disease states in a team-centered approach.
- Interacts with patients and families to keep them informed and engaged in their health care decisions.
- Provides counseling, education, and resource information on wellness, prevention, and health maintenance.
- Refers patients as appropriate for specialty care services and educational training.
- Uses disease registries to identify gaps and out-of-range values in care delivery.
- Participates in improving access to care through process improvement initiatives.
- Works with the process team to enhance care delivery efficiency in the office setting.
- Maintains, monitors, and ensures accurate and timely documentation of patient encounter records.
- Collaborates with Practice Development and Improvement Manager on achieving Patient-Centered Medical Home status and NCQA certification.
- Engages in clinical integration, including process improvement, coordination of care transitions, and communication with specialists and inpatient services.
- Follows up with patients post-discharge to ensure a smooth transition from inpatient to outpatient care.
- Strives to deliver value (quality/cost) to patients by providing timely and appropriate care.
Requirements
- M.D. or D.O. - Required
- Member of HonorHealth medical staff - Required
- Bilingual-Spanish - Preferred
- Basic Life Support (BLS) - Required
- Board Certified or eligible - Required
- Physician\MD - Physician - State Licensure - Required
- Fingerprint Clearance Card - State - Required
