Key Responsibilities
- Provide clinical evaluation, treatment, and follow-up care for residents.
- Perform rounds to observe residents, assess clinical needs, and ensure care plans are being followed.
- Manage acute and chronic conditions in collaboration with the interdisciplinary care team.
- Participate in admission decisions and resident status reviews as needed.
- Collaborate with nursing leadership, physicians, consultants, department heads, and community agencies to coordinate care and resolve clinical concerns.
- Communicate directly with residents, family members, medical staff, nursing staff, and members of the interdisciplinary team regarding care needs, changes in condition, and concerns.
- Support the development, implementation, and evaluation of resident care plans.
- Absorb and respond to resident incidents, unusual occurrences, and changes in condition.
- Participate in QAPI, facility assessments, performance improvement initiatives, and care-related committees as needed.
- Support infection control practices in accordance with current guidelines.
- Absorb and respond to survey preparation, mock surveys, plan of correction follow-up, and regulatory compliance efforts as needed.
- Promote safe work practices, teamwork, mutual respect, and effective communication.
- Provide direct clinical care as necessary.
- Support facility leadership in maintaining quality standards and improving overall resident outcomes.

Requirements
- Active and unrestricted license as a Physician or Nurse Practitioner in the practicing state.
- Experience in a nursing home, skilled nursing facility, hospital, or related healthcare setting preferred.
- Knowledge of medical and nursing practices, procedures, and regulations related to long-term care.
- Strong clinical assessment, documentation, communication, and leadership skills.
- Ability to work collaboratively with the Director of Nursing, Administrator, Medical Director, and interdisciplinary team.
- Current CPR certification preferred or as required by facility policy.
- Prior geriatric, skilled nursing, or long-term care experience is a plus.
