This position is for a Physician or an Advanced Practitioner at WI-St. Agnes Hospital Home Care & Hospice in Fond du Lac, WI.
Responsibilities
- Provide hospice and palliative care services to patients and families
- Collaborate with interdisciplinary team to develop and implement care plans
- Assess patient needs and provide medical management as needed
- Document patient care in accordance with regulatory requirements
Requirements
- Current medical license in good standing
- Experience in hospice and palliative care preferred
Qualifications
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
- Advanced Practice Registered Nurse (APRN) certification preferred
