Physician (MD/DO)

Position Overview

Key Responsibilities

• Oversee assigned patient’s care coordination including but not limited to specialty referral and follow up, case management, and enabling services.
• Provide professional medical services including examinations, diagnosis, and treatment planning for medical problems.
• Prepare and execute appropriate treatment plans and provide follow-up as needed while adhering to clinic policy.
• Maintain productivity expectations and procedures.
• Provide consultation when needed to nurse practitioners and physician assistants.
• Serve as supervising physician to no more than two mid-level providers.
• Proctor medical students as assigned.
• Maintain appropriate medical records documentation on all patients, in accordance with organizational policy.
• Meet expectations of above 90% on weekly documentation report and maintain PAQ at less than 75 items at any given period.
• Actively contribute to care team by providing coverage to absent providers and being available to support staff and serving on improvement teams.
• Actively engage in organizational initiatives such as 340b usage, Patient Centered Medical Home Certification, and Meaningful Use requirements.
• Work effectively in an integrated environment to coordinate care with dental and behavioral health professionals.

Qualifications

• MD/DO providers must have completed Primary Care residency (Family Practice, Internal Medicine, or Pediatrics) and be board certified.
• Licensed to practice in the State of Oregon with prescriptive authority and DEA.
• Ability and willingness to provide Obstetrical care preferred.
• Team-oriented with strong interpersonal and communication skills.
• Valid Oregon Driver's License and proof of automobile insurance.
• CPR certification in infant, child and adult required.
• Bilingual/bicultural strongly preferred.
• Previous experience in community health preferred.
• Ability to function independently.
• Ability to work flexible hours and take call.
