About the role

Medical Director Services PC (MDS PC) is a physician-owned group founded in 2016 that specializes exclusively in skilled nursing facility (SNF) medicine. The group focuses on treating patients in place, reducing avoidable hospitalizations, and maintaining close connections with the surrounding hospital network for continuity of care. This full-time Attending Physician position is based in Granville, NY.

Responsibilities

• Perform admissions, monthly, acute & routine visits for SNF residents on a regulatory and as‑needed basis
• Collaborate with APPs on your units
• Manage medication decisions — including narcotics and psychiatric medications — with an emphasis on reducing unnecessary polypharmacy
• Lead care planning, discharge planning, and ongoing staff education
• Support the facility through regulatory and survey site visits
• Coordinate with local hospitals to ensure continuity of care for residents

Requirements

• Active NYS medical license (MD or DO)
• Current DEA registration in good standing
• Up to date on CME requirements
• Experience in SNF, long‑term care, or managed care settings preferred
• Comfort working with a culturally and linguistically diverse resident population

Pay

Salary range: $150,000–$300,000+ per year
