Physician Leader - Primary Care

Responsibilities

• Act as on-site supervising Physician for assigned Advanced Practice Providers (APPs) and Physicians in the pod (collaborating physician). Ensure total number of collaborative practice agreements does not exceed state regulations.
• Collaborate with clinical team members associated with the pod, including Community Health Workers, Medical Social Workers, Nurse Navigators, Care Managers, and Patient Care Coordinators.
• Participate in at least one ride-along with all collaborative APPs per quarter and provide a complex patient visit as needed for each APP under supervision.
• Develop and participate in APP clinical development, focusing on evidenced-based care and professionalism.
• Lead and participate in high-risk huddles, ensuring all patients discussed have a documented plan of care that includes all care management team members.
• Build a team-based culture and work collaboratively with clinical team members.
• Conduct 10 chart audits per month or per state regulations, whichever is higher.
• Act as a resource for clinical team members for patient care questions or concerns and participate in difficult conversations with patients or family members as requested.
• Routinely conduct panel reviews with APPs and sign DME, death certificates, and other orders as needed on behalf of APPs (as regulated by state).
• Frequently review Quality metrics and pod-level performance, identifying areas for improvement to achieve superior Quality (HEDIS) performance.
• Review clinician capacity and productivity performance, partnering with Site Medical Directors to develop and implement countermeasures when providers are off track.
• Work collaboratively with the Regional Medical Director (RMD) to identify opportunities for care improvement.
• Assist with service inquiries, resolution of patient complaints, and requests for the pod as needed.
• Help pod providers achieve higher EMR efficiency.
• Participate in patient care for assigned panel and other patients as needed (e.g., high-risk or complex patients, physician face-to-face requirements, cognitive assessments).
• Lead clinical education regarding accurate diagnosis, documentation, and management of chronic conditions.
• Assist with compliance and HR resolutions, including discipline actions and performance reviews for direct reports.
• Perform additional duties as assigned by the Regional Medical Director related to clinical pod performance and outcomes.

Requirements

• Bachelor’s Degree
• Doctor of Medicine or Osteopathy
• Valid driver’s license and good driving record
• Active CPR Certification
• Unrestricted DEA
• Completion of an accredited Residency Program
• Unrestricted Medical License
• Board certification required for all newly hired or promoted physicians on or after January 1, 2026; must obtain and maintain certification in accordance with organizational and applicable regulatory standards.

Preferred Qualifications

• 3 years of experience as a Physician collaborating with Advanced Practice Providers
• Experience in Geriatrics or with complex patients
