Physician Leader, Primary Care

About the Role

Provide home-based primary and palliative care for medically complex, home-limited patients. Conduct in-home and virtual visits to assess and manage patient needs. Lead and coordinate interdisciplinary care teams to develop comprehensive care plans focused on reducing avoidable hospitalizations and enhancing patient quality of life. Champion a patient-first culture and mentor clinical staff while collaborating with nursing, social work, and care management teams to ensure seamless care delivery.
